Lykos Medical is seeking an experienced and compassionate Nurse Practitioner or Physician Assistant to provide mobile primary care services to seniors residing in assisted living communities across Colorado. This role is idóneo for clinicians who are ready to step away from the high-volume clinic or hospital setting and deliver deeply personalized care, one patient at a time. You’ll be a key player in a collaborative, mission-driven team that prioritizes connection, trust, and dignity in every patient interaction. What You’ll Do:
- Provide comprehensive in-person primary and geriatric care in assisted living communities
- Manage chronic conditions, conduct wellness and cognitive assessments, and provide urgent visits
- Collaborate with caregivers, families, and other healthcare providers for coordinated care
- Participate in transitional care planning and end-of-life support
- Accurately document visits using EMR and contribute to quality improvement efforts
- Serve as a clinical leader and advocate for best practices in senior care What We Offer:

- Licensed Nurse Practitioner or Physician Assistant in the state of Colorado
- 3+ years of clinical experience in internal medicine, geriatrics, or hospitalist settings required
- Strong ability to work independently in field-based environments
- Excellent clinical judgment, communication, and EMR documentation skills
